Items That Must Be On File in the Nursing Department

The following items must be on file in the nursing department for each instructor:

1.   Copy of current registered nurse’s license for the state of Georgia

2.   Completed Faculty Checklist with verification of RN license visualization, current dates of OSHA training and Blood Borne Instruction, liability insurance

3.   Current Faculty Health Evaluation Form and evidence of current PPD, tetanus and hepatitis vaccination

4.   Current American Heart Association BLS certification

5.   Current Faculty Supervision Applied Learning Experience Agreement
