CRITICAL ILLNESS BENEFIT
The critical illness benefit offered by UNUM provides a lump sum benefit payment to cover out-of-pocket medical expenses and the costs associated with life changes following a covered critical illness. The plan features a $10,000 employee/$5,000 spouse* benefit that is payable in a lump sum following the diagnosis of each covered critical illness. The following critical illnesses are covered by this benefit:
• Heart Attack • Major Organ Transplant
• Cancer • Kidney Failure (End Stage Renal)
• Stroke • Carcinoma in situ (25% of maximum benefit)
• Coronary Artery Bypass Surgery (25% of maximum benefit)
