CONTINUATION OF HEALTH/DENTAL COVERAGE
Under the Consolidated Omnibus Budget Reconciliation Act of 1986 (COBRA), upon termination, you or your covered dependents may have the option of continuing coverage under the health/dental benefits plan for a period of time by paying the required premiums. You or your dependents could require continuation of coverage due to your termination of employment, divorce, or your child no longer being eligible for coverage under the regular plan, due to age or absence of full time student status.
* The premium for COBRA coverage is 102% of the total premium (employee + employee portions).
ALL STATUS CHANGES SHOULD BE REPORTED IMMEDIATELY TO HUMAN RESOURCES!
