Recommended Actions:

The process of using the Accommodation Letter Request Form is successful in getting eligible students to request their accommodation letters within the first 30 days of the semester. The true problem is only 33 students of the eligible 72 students requested their accommodation letter. During fall and spring semesters 39 eligible students did not request accommodation letters and therefore should not have had accommodations in classes. The Access Center will stress the importance of the accommodation letter to faculty and request that faculty not accommodate without the accommodation letter from the Access Center.
